I/We authorize the Credit Union to obtain such factual and investigative information regarding me/us from others as permitted by law and to furnish other credit grantors and any credit bureau, particulars of this credit application. In addition, I/we hereby acknowledge notice from the Credit Union that a consumer report containing credit information may or will be referred to in connection with this application for credit or any renewal or extension thereof. I/We understand that I/we will be charged an administration fee of $15.00 at the time of closing the loan. This can either be paid in cash or it can be included in the loan amount.

I understand that to evaluate my credit application and to continue monitoring my credit status, and for the purposes I have consented to above, a Personal Information file containing credit and other personal information will be created. Only those employees of you and your Networking Affiliates or Cooperative Financial Services Group whose job functions involve assessment of credit worthiness, credit applications, monitoring, processing of payments and matters relating to the purposes consented to above, will have access to my file. If I wish to consult my file or make corrections to it, I may do so by written request to the Privacy Officer at 14 Chamberlain Avenue, Ottawa, Ontario, K1S 1V9
